iFUTBOL CAMP AGREEMENT & WAIVERS
MEDICAL INSURANCE INFORMATION: I understand that every participating individual is required to have health insurance coverage that provides an appropriate level of benefits befitting a participant in a contact sport such as Soccer. I affirm that my child has an appropriate level of coverage.
RELEASE OF LIABILITY: I hereby release and discharge, indemnify and hold harmless iFutbol Camp and their members, officer, employees and any other individuals or entities acting on the behalf and the successors and assigns for any and all of the aforementioned individuals and entities, against all claims, demands, cost and expenses, and cause of action whatsoever, either in law or equity, arising out of or in any way connected with any property loss and or bodily injury and or disability, arising from my child’s participation in the camp activities.
CONSENT FOR TREATMENT: I hereby give my permission to a camp certified athletic trainer to supervise on-site first aid for minor injuries. For more serious injuries we will contact the parent or guardian to inform you of the need for any medical attention beyond minor first aid, if necessary.
PHYSICAL EXAMINATION WITHIN ONE YEAR: I certify that within the past 12 months my child has had a physical examination by a physician and that he is physically able to participate in the sports camp activities.
ASSUMPTION OF FINANCIAL RESPONSIBILITY: I hereby acknowledge that I am responsible for medical charges incurred during all camp participation. I further understand that iFutbol Camp carries an excess medical insurance policy for sports injuries to the camper that may result from camp activities. This policy may only be utilized after my primary insurance company has processed the claims and issued an explanation of benefits.
UNDERSTANDING OF RISK OF CONCUSSION AND TRAUMATIC BRAIN INJURY
What is a concussion? A concussion is a brain injury that:
Is caused by a bump, blow, or jolt to the head or body. Can change the way an individual’s brain normally works. Can occur during Practices and/or Contests in any sport. Can happen even if an individual has not lost consciousness.
Can be serious even if an individual has just been “dinged” or “had their bell rung.”
All concussions are serious. A concussion can affect an individual’s ability to do schoolwork and other activities (such as playing video games, working on a computer, studying, driving, or exercising). Most people with a concussion get better, but it is important to give the concussed individual’s brain time to heal.
What are the symptoms of a concussion?
- Concussions cannot be seen; however, in a potentially concussed individual, one or more of the symptoms listed below may become apparent and/or that the individual “doesn’t feel right” soon after, a few days after, or even weeks after the injury.
- Headache or “pressure” in head Nausea or vomiting
- Balance problems or dizziness
- Double or blurry vision
- Feeling sluggish, hazy, foggy, or groggy
- Difficulty paying attention
- Memory problems
- Confusion Bothered by light or noise
What should individuals do if they believe that they or someone else may have a concussion?
- Individuals feeling any of the symptoms set forth above should immediately tell their Coach and their parents.
- Also, if they notice any teammate evidencing such symptoms, they should immediately tell their Coach.
- The individual should be evaluated. A licensed physician of medicine or osteopathic medicine (MD or DO), sufficiently familiar with current concussion management, should examine the individual, determine whether the individual has a concussion, and determine when the individual is cleared to return to participate in soccer camp.
Concussed people should give themselves time to get better. If an individual has sustained a concussion, the individual’s brain needs time to heal. While a concussed individual’s brain is still healing, that individual is much more likely to have another concussion.
Repeat concussions can increase the time it takes for an already concussed individual to recover and may cause more damage to that individual’s brain. Such damage can have long term consequences. It is important that a concussed individual rest and not return to play until the individual receives permission from an MD or DO, sufficiently familiar with current concussion management, that the individual is symptom-free.
How can individuals prevent a concussion? Every sport is different, but there are steps individuals can take to protect themselves.
Follow the Coach’s rules for safety and the rules of the sport. Practice good sportsmanship at all times.
If an individual believes they may have a concussion: Don’t hide it. Report it. Take time to recover.
I hereby acknowledge that I am familiar with the nature and risk of concussion and traumatic brain injury while participating in athletics, including the risks associated with continuing to compete after a concussion or traumatic brain injury.
Covid-19 Waiver of Liability
Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19
The novel coronavirus (“COVID-19”) has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. iFutbol Camp has put in place preventative measures to reduce the spread of COVID-19; however, cannot guarantee that you will not become infected with COVID-19. Further, attending soccer camp at iFutbol camp could increase your risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 while attending iFutbol Camp and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at iFutbol Camp may result from the actions, omissions, or negligence of myself and others, including, but not limited to, iFutbol Camp employees, volunteers, and camp participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at iFutbol Camp or(“Claims”). On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless iFutbol Camp, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of iFutbol Camp, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after attending any iFutbol Camp program or activity.
